It depends on the cause and severity of being underweight. Very severe nutritional causes of being underweight are rare in the U.S. and thus not a problem. However, people who become anorexic (lose excessive amounts of weight because of body image problems) often do have heart problems resulting in a sudden abnormal heart rhythm, which can be fatal.
